Referring Vets

The form below is to be filled out by the referring veterinarian.

Our referral form asks specifically about problems, previous treatments, and the direction of care intended for your patient.

Horton Discovery is here to help 24 hours a day, 7 days a week. We are here overnight to support your hospital. Your patient will remain your patient. Horton doctors and staff will communicate with any referring veterinarian judiciously during the treatment of a referred case. Please provide your direct phone number so we can give you updates regarding changes even in the middle of the night. As a referring veterinarian, you can call our hospital at (573) 777-3609 to discuss or follow up on any transferred case. Call for the email if needing to send records.

We would like to transfer your case back in the morning if that works well for your referral hospital and the patient.
